Application to excavate within the road right-of-way or a Miami Township road is hereby made by:
All work will be done in accordance with the rules and conditions set forth by the Public Works Department. All work will be inspected by a representative of the Public Works Department. Any work that fails to pass inspection will be removed and replaced at the applicant’s expense until said work does pass inspection. Traffic will be maintained through the work zone at all times unless permission has been granted by the Public Works Department to close the road. A performance bond in the amount of not less than $20.00 per linear foot will be posted for this work. This bond will be held for nine months in all cases that involves open cutting of the roadway, in all other cases the bond will be released upon passing the final inspection. Liability insurance must be maintained by the applicant for the duration of the work. Applicant assumes all responsibility for personal injury and property damage claims that might arise from this work. Applicant shall be responsible for notifying OUPS (Ohio Utilities Protection Services) prior to excavations within the road right-of-way when necessary.
To complete permit form on-line, the applicant must have current proof of insurance and performance bond information on file at the Public Works offices. The applicant must complete the form at the Public Works offices if submitting payment by cash or certified check with permit request. The Miami Township inspector will notify the applicant upon verification of information to discuss the inspection schedule. No work shall commence until the inspector has granted approval to proceed with work.
